Abstract Case report A 43-year-old woman presented with acute onset migratory right iliac fossa pain, on a background of intermittently painful groin swelling. CT demonstrated differential diagnosis appendicitis besides an ovarian cyst. Diagnostic laparoscopy was undertaken view to and treat possible appendicitis, cyst inguinal hernia. To our surprise, haemorrhagic nuchal seen intraoperatively, duly aspirated. Post-operatively she recovered well discharged the same day. Introduction The Canal Nuck is small evagination parietal peritoneum which accompanies round ligament through canal, in females. It obliterated first year life. When this remains patent, it can fill fluid result formation cysts (akin hydroceles patent processus vaginalis). In current literature there are only sixteen case reports adults, one reported.
